 Ruth advised that Christine did not want to have any details of her diagnosis but she is aware that she has memory issues.
 
 

 Can get confused with names.
 Cognition deteriorated after a hospital admission but has shown improvement when she came home from but has plateaued. Happy in herself. 

 Converses well when engaged in conversation.

 Feb/March 25 - UTI admitted to hosp, delirium. Lost mobility but has improved.
 Uses wheelchair when out - lost confidence after being poorly this year. 
 Cognition
 Has broken both elbows over the years so has limited movement in them.
 No concerns with personal care. Will use incontinence underwear as a precaution and to increase confidence when out. 

·        Christine uses ready meals and can prepare drink - no concerns.

·        Private carers 2 x weekly - Ruth supports other days. 
 Enjoys the company of the careers and family. 

·        LPA held by Ruth and her brother.
